Return Material Authorization (RMA) Form

To expediate your repair or return, please complete this form in its entirety. 

Our process is as follows:

  1. Complete this request and submit to Control Solutions LLC for review.

  2. You will be notified within 3 – 5 business days of any potential credit and provided an RMA number.

  3. Only product with an approved RMA number may be returned for credit.

  4. Credit is issued only after receipt and favorable inspection.
